On June 15, 2022, Health Plan of San Mateo (HPSM) became aware of a data security incident. The breach is related to OneTouchPoint, a mailing vendor used by Matrix Medical. OneTouchPoint identified unauthorized access to their servers on April 27, 2022.
On a second privacy incident in January 2023, HPSM became aware of unauthorized access within their employee email environment. An unauthorized person gained access to one email account on January 17, 2023.
According to San Mateo, the information involved may have included the following:

- Date of birth
- Health information
