Horizon Actuarial Services has announced breaches of protected health information of up to 194,000 plan members. Horizon Actuarial announced the security breach and the theft of sensitive personal data of members of benefits plans to whom it provides technical and actuarial consulting services.

Horizon Actuarial was notified in November 2021 by a potential ransomware cybercriminal that claimed to have stolen the personal data of Horizon plan members. Horizon Actuarial confirmed that two servers had been hacked and files were compromised, including:

  • Names
  • Dates of birth
  • Social Security numbers
  • Health plan information
