TriValley Primary Care has begun sending out letters that indicate that in October 2021, a ransomware incident impacted company networks and servers. Over 122,000 individuals were allegedly impacted by the data breach.

- Prescription information
- Provider name
- Medical account number

- Health insurance policy group plan number
