The Urology Center of Colorado recently discovered that parts of its computer network may have been hacked and accessed by an unauthorized individual, compromising 137,820 individuals. A security breach was detected on September 8, 2021, while the data breach investigation confirmed the attack the previous day.
The compromised network may have contained patient data, including the following types of personal information:
- Name
- Date of birth
- Social Security number
- Address
- Phone number
- Email address
- Medical record number
- Diagnosis
- Insurance provider
