According to the Department of Health and Human Services, Wisconsin-based Forefront Management, LLC and Forefront Dermatology discovered a security breach in June 2021, potentially compromising the personal information of over 2 million individuals.

The Forefront Dermatology data breach incident allegedly occurred when unauthorized individuals gained access to their network, potentially exposing employee and patient information. The following personal information may have been leaked:

  • Names
  • Social Security Numbers
  • Addresses
  • Dates of birth
  • Patient account numbers
  • Health insurance member ID numbers
  • Medical record numbers
  • Dates of service & provider names
  • Medical and clinical treatment information

The Forefront Dermatology hacked systems were immediately taken offline in an attempt to prevent further damage, and an investigation to determine the full scope of the attack is underway.
