Gateway Rehabilitation Center, a company run out of Pennsylvania, has begun sending out data breach notice letters after a data security incident that may have impacted the personal information of thousands of patients.
On June 13, 2022, Gateway Rehab discovered a data security incident, and a subsequent investigation found data had been accessed and potentially removed from its systems.
Up to 130,000 individuals may be impacted. The following information may have been compromised in the incident:
- Name
- Date of birth
- Social Security number
- Driver’s license or state ID number
- Financial account and/or payment card number
- Medical information
- Health insurance information
