BakoDx discovered in December 2021 that certain personal information may have been accessed and removed from its computer network following unauthorized activity. The resulting investigation determined that an unauthorized third party accessed certain systems that contained personal information and stole some data between December 21 and 28, 2021.

Bako Diagnostics, also known as BakoDx, provides various laboratory services focusing on skin, tissue and bone to health care providers. Based on the data breach incident investigation, it was determined that the following information may have been stolen:

Name
Date of birth
Address
Social Security number
Driver’s license number
Email Address
Health Insurance information
Medical information
Financial account information
