The Food and Drug Administration (FDA) has warned consumers not to purchase or serve Once Upon a Farm Tractor Wheels Organic Toddler Soft-Baked Barsafter receiving reports of toddlers experiencing serious choking incidents, including complete airway obstruction. The bars have been recalled and our product recall attorneys are investigating related injuries.

The FDA issued its initial warning on October 30, 2026, after receiving several reports involving children between approximately 18 months and two-and-a-half years old.

“The FDA has received several reports that describe toddlers (ages 1.5–2.5 years) suffering complete airway obstruction with the inability to breathe after consuming the product,” the advisory says. “When chewed, the product reportedly forms a dry, sticky mass that standard choking interventions—including back blows and the Heimlich maneuver—may fail to dislodge. Some reported choking incidents have required emergency response intervention and hospital observation.”

Once Upon a Farm subsequently announced a voluntary recall covering all three flavors of its Tractor Wheels bars.

The affected products include Apple, Sweet Potato & Spinach; Strawberry, Pumpkin & Beet; and Banana, Pumpkin & Cauliflower. The recall applies to all lot codes, expiration dates, quantities, and varieties, including products with expiration dates through August 26, 2027.

The products were distributed nationwide and in Puerto Rico through retail stores and online retailers. According to the FDA, affected products were sold at Costco, Target, and Walmart, among other outlets, and may also have been available through Amazon and third-party sellers.

Consumers are advised not to serve any of the recalled bars and to return them for a refund or dispose of them immediately.

ABC News reported that Once Upon a Farm received three choking-related reports.

‍The company has acknowledged these safety concerns and stated that it was evaluating and reformulating its Tractor Wheels products. The company also expressed regret, saying that it had fallen short of the expectations families place in its products.

Once Upon a Farm Recall Raises Questions About Consumer Safety

A product recall does not automatically establish legal liability. However, serious injuries associated with a recalled product can raise important questions about product design and age-appropriate safety warnings.

Food products marketed specifically to toddlers warrant particular attention because young children may face unique risks when consuming certain textures and food consistencies.

Families whose children experienced choking or other complications after consuming these products may wish to preserve the packaging and medical records. These materials can help establish what product was consumed and the nature of the resulting injuries.

How a Product Recall Lawyer Can Help

A product recall attorney can investigate whether a product may have been defectively designed or manufactured, whether adequate safety testing was performed, and whether consumers received appropriate warnings. Depending on the circumstances, injured families may have grounds to pursue compensation for medical expenses and damages associated with their child's injuries.
